Verificar, nas evidências, estratégias que podem ser utilizadas para garantia da continuidade do cuidado na Atenção Primária à Saúde após a alta hospitalar. Trata-se de uma Revisão Integrativa da Literatura. A coleta de dados entre agosto e setembro de 2022 nas bases de dados Literatura Latino-Americana e do Caribe em Ciências da Saúde, na Biblioteca Virtual em Saúde e na National Library of Medicine. Foram incluídos 16 artigos. As possíveis estratégias encontradas foram: existência de um enfermeiro de ligação, de enlace ou gerente casos, resumo ou relatório de alta, planejamento da alta hospitalar, sistemas informatizados em saúde integrados e protocolos institucionais. Verificou-se possíveis estratégias a serem implementadas para a garantia da continuidade do cuidado após a alta hospitalar. É perceptível a fragilidade de ações nessa temática no cenário brasileiro, sendo mais evidenciada em Espanha e Canadá.

 Continuity of care as a strategy for comprehensive health care
 Abstract: To verify, in the evidence, strategies that can be used to guarantee the continuity of care in Primary Health Care after hospital discharge. This is an Integrative Literature Review. Data collection between August and September 2022 in the databases Latin American and Caribbean Literature in Health Sciences, the Virtual Health Library and the National Library of Medicine. 16 articles were included. The possible strategies found were: existence of a liaison nurse, liaison nurse or case manager, discharge summary or report, hospital discharge planning, integrated computerized health systems and institutional protocols. It was verified possible strategies to be implemented to ensure continuity of care after hospital discharge. It is noticeable the fragility of actions in this theme in the Brazilian scenario, being more evident in Spain and Canada.

 Resumen: Verificar, en la evidencia, las estrategias que pueden ser utilizadas para garantizar la continuidad de la atención en la Atención Primaria de Salud después del alta hospitalaria. Se trata de una Revisión Integrativa de la Literatura. Recolección de datos entre agosto y septiembre de 2022 en las bases de datos Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Biblioteca Virtual en Salud y Biblioteca Nacional de Medicina. Se incluyeron 16 artículos. Las posibles estrategias encontradas fueron: existencia de una enfermera de enlace, enfermera de enlace o administrador de casos, resumen o informe de alta, planificación del alta hospitalaria, sistemas de salud computarizados integrados y protocolos institucionales. Se verificaron posibles estrategias a implementar para asegurar la continuidad de la atención después del alta hospitalaria. Es notable la fragilidad de las acciones en este tema en el escenario brasileño, siendo más evidente en España y Canadá.
