Resumo Resultado direto da Reforma Psiquiátrica no Brasil, os Centros de Atenção Psicossocial (CAPS) são serviços de saúde de caráter comunitário destinados a indivíduos diagnosticados com transtornos mentais severos e/ou persistentes. Partindo da abordagem de Erving Goffman, o objetivo deste artigo é analisar os efeitos práticos do modo de funcionamento dos CAPS, mais especificamente da eliminação do caráter compulsório do tratamento e da afirmação da participação voluntária na dinâmica dos ajustamentos primários e secundários dos usuários à instituição. A análise parte de uma pesquisa realizada em um CAPS da cidade de São Paulo entre 2014 e 2016 e indica que o caráter voluntário da participação no CAPS não se traduz necessariamente em adesão à instituição e não elimina o caráter problemático da cooperação dos usuários com a rotina proposta. O ajustamento à concepção de si vinculada aos sintomas mentais permanece um processo conflitivo.
