POINTS CLÉSÀ quelques exceptions près, une infection par Mycobacterium tuberculosis (M. tuberculosis) s’acquiert par inhalation de microgouttelettes (1 à 5 micromètres de diamètre) ne contenant que quelques mycobactéries capables d’atteindre les alvéoles pulmonaires.Les facteurs suivants augmentent la probabilité de transmission:charge bacillaire (frottis positif) du cas source;patientTB cavitaire ou TB pulmonaire siégeant dans la partie supérieure des poumons, comme montré par la radiographie thoracique du cas source;maladie laryngée du cas source;fréquence et sévérité de la toux chez le cas source;durée de l’exposition;proximité du cas source;espaces surpeuplés et mal ventilés;retard dans le diagnostic ou dans la mise en route d’un traitement efficace du cas source.La manière la plus efficace de réduire la transmission est de diagnostiquer rapidement la maladie et de traiter les patients atteints d’une TB pulmonaire active le plus rapidement possible.Chez certaines personnes, les macrophages alvéolaires éliminent les bacilles par des mécanismes immunitaires innés; chez d’autres, les bacilles peuvent se multiplier et provoquer une infection de TB. Les facteurs liés aux bacilles et les facteurs génétiques liés à l’hôte qui favorisent ou limitent l’acquisition de l’infection ne sont pas bien compris.Deux états intermédiaires récemment décrits surviennent entre le moment de l’infection et le développement d’une TB pulmonaire active et symptomatique: la tuberculose incipiens, un état susceptible d’évoluer vers une maladie active, mais ne provoquant pas d’anomalies détectables; et la tuberculose infraclinique, un état de maladie dû à une infection viable par M. tuberculosis ne causant pas de symptômes cliniques liés à la tuberculose, mais pouvant être détecté à l’aide des tests radiologiques ou microbiologiques existants.
