Hemophagocytic Lymphohistiocytosis in an Otherwise Healthy Traveller

Abstract
We report a case of hemophagocytic lymphohistiocytosis (HLH) in an otherwise healthy man who presented with a subacute febrile illness after travelling to Arizona. No underlying precipitant for his HLH was identified, nor did he have any pathogenic sequence variants from the established HLH genetics panel. His clinical and laboratory manifestations of HLH resolved with steroid therapy and one dose of etoposide, as per the HLH-94 protocol, and he was discharged clinically well. Given its high rate of mortality and its variety of clinical presentations, it is crucial that practitioners in all settings and specialties have awareness of and maintain a high index of suspicion for HLH to facilitate early recognition of the disease and improve patient outcomes.
