Introduction: The diagnosis of milk and other food protein allergies is often difficult to confirm in non type I (IgE negative) reactions. Reliance on results of food exclusion diets is often misleading and placebo controlled food allergen challenges are impractical. Aim: To establish a reproducible, practical and reliable in vitro cytokine assay using peripheral blood as a potential diagnostic tool for food allergies. Methods: Heparinized whole blood was diluted (1:1) in RPMI 1640 medium. For mitogen control wells, PMA and calcium ionophore were added. For allergen stimulation, either Betalactoglobulin (BetaLG), casein (CAS) or ovalbumin (OVA), each at 500mug/ml, was added to separate wells in duplicate for 24, 48 or 72 hours. Subsequently, a protein transport inhibitor (Golgi-Stop) was added for the final 4 hours of incubation. At the end of the stimulation culture period, erythrocytes were then lysed and the remaining cells were resuspended in PBS with 1% BSA. Cell surface staining was carried out using antibodies to CD4 and CD8. The cells were then suspended in a fixative (Cytofix/Cytoperm), and washed. Intracellular cytokine straining was then carried out using specific antibodies to IL-4, Il-5, Il-13, IFNgamma, and TNFalpha. In order to evaluate T cell activation, FastImmune CD69/CD3 was used in separate wells. Results were then analyzed by flow cytometry. Results: Optimal results were obtained at 24 hours, with increased markers of T cell activation (CD69+/CD3+) and cytokine staining in response to BetaLG (representative experiment shown in Table). Conclusion: Intracellular staining of T cells derived from whole blood with autologous serum after stimulation in vitro with food allergens for 24 hours is a reproducible and practical method to examine food hypersensitivity in vitro. The clinical reliability of this test in terms of predicting food challenge results is under study.
