Het Belgische model van 'integrale' levenseindezorg bestaat sinds 2002 uit het samengaan van universele toegang tot palliatieve zorg (PZ) en wettelijk geregelde euthanasie. Als eerste organisatie wereldwijd heeft de Federatie voor PZ Vlaanderen (FPZV) euthanasie ingebed in de praktijk van PZ. Dit model kreeg grote bijval (het is recentelijk door Québec, Canada, goeddeels overgenomen), maar ook heftige kritiek. Volgens buitenlandse critici gaan concepten zoals 'integrale PZ' en 'palliatieve futiliteit of hardnekkigheid' fundamenteel in tegen de essentie van PZ. Dit artikel analyseert enkele essentialistische argumenten voor de vermeende incompatibiliteit tussen euthanasie en PZ. <BR>Grootschalige empirische studies uit de Benelux tonen dat sinds de legalisering van euthanasie de zorgvuldigheid van levenseindebeslissingen is toegenomen en dat zich geen significante 'hellend vlak'-effecten hebben voorgedaan.<BR> Het is problematisch dat sommige critici deze proefondervindelijke bewijzen afdoen als irrelevant in een normatief debat. Het historisch-conservatieve argument tegen het Belgische model dat de preventie van euthanasie een stichtend principe van de PZ was, negeert belangrijke historische ontwikkelingen. Het ethische standpunt van conservatieve critici doet afbreuk aan het kernidee van PZ dat de patiënt centraal staat, aangezien de voorkeuren van zorgverleners boven die van patiënten worden geplaatst. Verder is het canonieke vasthouden van vele critici aan de definitie van PZ van de Wereldgezondheidsorganisatie (WGO), met <I>intentie </I>als centraal criterium, problematisch omdat intentie niet deugdelijk is als ethische maatstaf. <BR> Een dogmatische afwijzing van het Belgische model kan ook nefaste <I>praktische </I>gevolgen hebben. Hierbij hoort de marginalisering van PZ in euthanasie-permissieve landen, de verderzetting van clandestiene praktijken en problematische sedatie tot aan het overlijden. De algehele kwaliteit van levenseindezorg zou achteruitgaan omdat 1) patiënten die op de <I>mogelijkheid </I>van euthanasie staan zich ver zouden houden van professionele PZ, en dus minder competente levenseindezorg zouden krijgen en 2) euthanasie minder eu-euthanasie zou zijn, wegens minder uitgevoerd in een geest van 'totale zorg'. <BR>Samengevat, de essentialistische argumenten tegen het Belgische model negeren empirische studies, hanteren problematische definities, onderschikken de waarden van de patiënt aan die van de zorgverlener en miskennen een pluraliteit van verdedigbare perspectieven over levenseindebeslissingen.<BR><HR> <B>Euthanasia embedded in palliative care </B><BR>The Belgian model of ‘integral’ end-of-life care consists of universal access to palliative care (PC) and legally regulated euthanasia. As a first worldwide, the Flemish PC organisation has embedded euthanasia in its practice. However, some critics have declared the Belgian-model concepts of ‘integral PC’ and ‘palliative futility’ to fundamentally contradict the essence of PC. This article analyses the various essentialistic arguments for the incompatibility of euthanasia and PC.<BR> The empirical evidence from the euthanasia- permissive Benelux countries shows that since legalisation, carefulness (of decision making) at the end of life has improved and there have been no significant adverse ‘slippery slope’ effects. It is problematic that some critics disregard the empirical evidence as epistemologically irrelevant in a normative ethical debate. Next, rejecting euthanasia because its prevention was a founding principle of PC ignores historical developments. Further, critics’ ethical positions depart from the PC tenet of patient centeredness by prioritising caregivers’ values over patients’ values. <BR>Also, many critics’ canonical adherence to the WHO definition of PC, which has intention as the ethical criterion is objectionable. A rejection of the Belgian model on doctrinal grounds also has nefarious practical consequences such as the marginalisation of PC in euthanasia-permissive countries, the continuation of clandestine practices and problematic palliative sedation until death. <BR>In conclusion, major flaws of essentialistic arguments against the Belgian model include the disregard of empirical evidence, appeals to canonical and questionable definitions, prioritisation of caregiver perspectives over those of patients and rejection of a plurality of respectable views on decision making at the end of life.
