Abstract
L'évaluation médico-économiques des maladies chroniques et handicapantes, telles que la polyarthrite rhumatoïde (PR), est complexe et l'évolution des modalités thérapeutiques rendent nécessaires de nouvelles évaluations. Ce travail de thèse a permis de produire de nouveaux éléments de réponse sur : 1 -les coûts diagnostiques de la PR, au moyen d'une enquête de pratique. Le principal déterminant de ceux-ci était le degré d'incertitude du médecin; 2 -les coûts d'utilisation des nouvelles biothérapies, efficaces mais coûteuses. Cette étude de minimisation de coûts a montré que les modalités de suivi des patients constituaient la principale source d'hétérogénéité des coûts; 3 -la méthode de valorisation contingente au travers d'une enquête téléphonique explorant la propension à payer (PAP) de la population générale et de patients pour un traitement curatif de la PR en France et au Canada. Une certaine concordance des résultats a été observée en France et au Canada, mais plusieurs biais ont été observés, liés au format de l'étude et aux caractéristiques des répondants. Malgré ces limites, les études de PAP semblent capables d'impliquer des non experts dans une réflexion sur la santé et ses coûts.
