Complications from diabetes result in significant challenges for individuals, families and health-care systems. However, there are successful evidence-based solutions to prevent four out of five amputations. The Foot Health Pathway presented here—which is based on an approach that has been successfully implemented in other countries—provides a framework that could be used for optimized care delivery in Canada.
