Abstract
Contexte : Les nouveaux vaccins conjugués 15 et 20-valents contre le pneumocoque (PCV15 et PCV20) ont été mis sur le marché sur la base de critères d'immunogénicité, chacun démontrant une non-infériorité en ce qui concerne la réponse par rapport au vaccin 13-valent (PCV13).Dans le passé, le PCV13 a également été autorisé sur la base des mêmes critères, en utilisant le vaccin 7-valent (PCV7) comme référence.Méthodes : Notre objectif était de comparer l'immunogénicité de ces trois vaccins chez les bambins.Les ratios des titres d'activité opsonophagocytaire fonctionnelle (OPA) mesurés dans les mêmes essais randomisés et dans des essais différents ont été calculés pour évaluer l'immunogénicité respective de ces quatre produits.Résultats : Les résultats suggèrent que le PCV15 et le PCV20 sont moins immunogènes que le PCV13 pour les sérotypes les plus courants et que les deux nouveaux vaccins induisent une réponse largement comparable.Le vaccin PCV7 était déjà légèrement plus immunogène que le PCV13, ce qui signifie que les vaccins PCV15 et PCV20 soutiennent mal la comparaison avec le PCV7.Les résultats indiquent également une immunogénicité réduite du schéma 2+1 doses par rapport à celle du schéma 3+1 doses pour le PCV13, le PCV15 et le PCV20.Conclusion : Des études devront être menées à la suite de la commercialisation pour évaluer l'efficacité clinique du PCV15 et du PCV20 et leur avantage en vie réelle par rapport au PCV13.
