Von Willebrand factor (VWF) is synthesized in two cell types, endothelial cells and megakaryocytes. The regulation of VWF expression involves influences at various stages in its biosynthetic pathway starting with transcriptional activation and ending with complex protein modifications during its transit through the endoplasmic reticulum and Golgi. In this review, we summarize details of the transcriptional modulation of VWF describing the physiologic and pathologic mechanisms responsible for controlling VWF mRNA production. To date, there are several endothelial cell-specific transcription factors that have been demonstrated to either activate or suppress VWF expression. The sites of interaction of these proteins with cis-acting elements in the VWF regulatory sequence have been identified and functional evidence for their activity has been presented. However, the molecular signals that mediate these interactions remain largely unknown and require more investigation.
