Abstract
Since the identification of accelerated von Willebrand factor (VWF) clearance as a pathogenic mechanism in type 1 von Willebrand disease (VWD), the complex topic of VWF clearance has been the focus of intense investigation. Large observational genetic studies, in tandem with complementary cell and animal-based research, have greatly expanded our knowledge of the biological and molecular mechanisms involved. Macrophages and sinusoidal endothelial cells have emerged as key mediators of both physiological and pathological VWF clearance and have been shown to express an array of lectin and scavenger receptors that differentially bind to and endocytose plasma VWF based on polypeptide or glycan affinity. This chapter details the recent advances and current knowledge of VWF clearance pathways, with a particular focus on genetic, molecular, and cellular contributions.
