Kortikosteroid-Anwendung und OP-Vorgeschichte zu Studienbeginn bei Patienten mit chronischer Rhinosinusitis mit Nasenpolypen im globalen Register AROMA

Abstract
Einleitung AROMA (NCT04959448) ist eine prospektive globale Registerstudie zur Beschreibung der Dupilumab-Behandlung bei Patienten mit chronischer Rhinosinusitis mit Nasenpolypen (CRSwNP) unter Alltagsbedingungen. Diese Analyse berichtet über Kortikosteroid-Anwendung und OP-Vorgeschichte zu Studienbeginn. Methode In das Register AROMA werden Erwachsene mit CRSwNP zu Beginn ihrer Dupilumab-Behandlung aufgenommen und bis zu 36 Monate beobachtet. Zu Studienbeginn werden demografische Daten, Krankheitsmerkmale sowie die medizinische und chirurgische Vorgeschichte erfasst. Ergebnisse Bis Februar 2023 wurden 303 Patienten aufgenommen. Das mittlere (SD) Alter betrug 50,8 (13,4) Jahre; 49,5% waren männlich. 68,6% der Patienten gaben an, in den 24 Monaten vor der Aufnahme orale/systemische Kortikosteroide angewandt zu haben. Bei 45,2% waren Antibiotika gegen CRSwNP verordnet worden. In den 12 Monaten vor der Aufnahme erhielten 103 Patienten Prednison, 26 erhielten Methylprednisolon und 28 erhielten Prednisolon. Die mittleren (SD) kumulativen Dosen oraler/systemischer Kortikosteroide in den letzten 12 Monaten betrugen 110,6 mg (260,4) bei Prednison, 67,0 mg (193,9) bei Methylprednisolon und 272,7 mg (379,0) bei Prednisolon. 187 (61,7%) der Patienten waren bereits an den Nasennebenhöhen operiert worden; bei 69 (22,8%) waren es zwei oder mehr Eingriffe. 116 (38,3%) waren nicht voroperiert. Schlussfolgerung Bei Erwachsenen mit CRSwNP, die eine Dupilumab-Behandlung beginnen, ist die Belastung mit oralen/systemischen Kortikosteroiden und Antibiotika hoch.
