Abstract
Inhibitory immunologicznych punktów kontrolnych (ICI) są zatwierdzoną terapią pierwszego rzutu w leczeniu wielu nowotworów i znacząco poprawiają rokowanie pacjentów.Niestety, mogą one powodować powikłania, wśród których powikłania ze strony serca są wprawdzie rzadkie, lecz wysoce śmiertelne.Wczesne wykrycie i leczenie mają kluczowe znaczenie dla zmniejszenia śmiertelności, dlatego wiedza na temat objawów kardiologicznych powikłań tych terapii jest bardzo istotna.Najczęstszym kardiologicznym zdarzeniem niepożądanym jest zapalenie mięśnia sercowego.Inne powikłania obejmują kardiomiopatię takotsubo i rozstrzeń lewej komory, zapalenie osierdzia, wysięk osierdziowy, tamponadę serca, arytmię, olbrzymiokomórkowe zapalenie tętnic i miażdżycę.Częstość występowania tych powikłań waha się od 3,1% w przypadku stosowania monoterapii do 5,8% w przypadku terapii skojarzonej.Śmiertelność sięga 50% w przypadku zapalenia mięśnia sercowego i 26% w przypadku arytmii.Najbardziej znanym czynnikiem predykcyjnym powikłań kardiologicznych jest terapia skojarzona.Inne czynniki ryzyka obejmują starszy wiek, płeć męską, rasę czarną i choroby układu sercowo-naczyniowego.
