Connectivité fonctionnelle de l’hippocampe antérieur et postérieur dans l’épilepsie focale pharmaco-résistante

Dans le cadre d’un bilan pré-chirurgical pour des patients atteints d’épilepsie focale pharmaco-résistante (EFPR), des électrodes intracérébrales (stéréo-électroencéphalographie, SEEG) sont implantées permettant la stimulation intracrânienne et l’enregistrement des réponses cérébrales associées. Elles permettent de cibler la zone épileptogène (ZE) pour sa future résection. Cette chirurgie cible souvent l’hippocampe (HPC) et l’étude des profils de connectivités fonctionnelles permettra d’évaluer les potentiels effets délétères. Plusieurs études d’Imagerie par Résonance Magnétique fonctionnelle (IRMf) ont observé des profils de connectivité distincts pour l’hippocampe antérieur (HPCa) et postérieur (HPCp). La présente étude contribue à préciser cette distinction par une mesure directe de l’activité intracrânienne (SEEG) chez cinq patients atteints d’EFPR. Les données de SEEG ont été analysées pour extraire les aires cérébrales se synchronisant avec la stimulation électrique. Les résultats confirment partiellement les profils distincts observés en IRMf : l'HPCa se synchronise avec le gyrus frontal inférieur, le gyrus lingual et le lobe temporal et l'HPCp avec le gyrus cingulaire, le pôle temporal, l’opercule rolandique, le cortex préfrontal ventromédian et le lobe pariétal inférieur. Nos résultats montrant des profils de connectivités fonctionnelles distincts suggèrent qu’une résection de l'HPCa et l'HPCp pourraient entraîner des conséquences différentes sur le fonctionnement cognitif des patients post-chirurgie de l’épilepsie.
