Abstract
The Vancouver classification categorizes periprosthetic fractures about a hip arthroplasty according to the site of the fracture, the stability of the implant, and the integrity of the surrounding bone stock. 1 Fracture location dictates the type: A, proximal to the prosthesis; B, including or just below the prosthesis; and C, well below the stem tip. Subtypes are labeled 1 for a stable prosthesis and 2 for an unstable prosthesis. If bone stock is compromised by osteopenia, osteolysis, or severe comminution, then subtype 3 applies. This classification system is both reliable and valid when used by experts and nonexperts. 1 The greatest value of this classification system is that it guides treatment decisions that will optimize outcome.
