Abstract
La littératie en santé (LS) mesure la compréhension de l’information médicale. Une LS élevée est liée à une moindre probabilité de rejet de greffe. L’éducation thérapeutique du patient (ETP) favorise l’autonomie, mais une faible LS peut entraver cet apprentissage. Cette étude vise à déterminer si les niveaux de LS varient selon les organes transplantés (rein, foie, poumon), en examinant également l’évolution de cette LS après les séances d’ETP. L’objectif : – Principal : évaluer et comparer les niveaux de LS chez des patients ayant reçu différentes transplantations d’organes avant l’ETP et deux mois après la sortie d’hospitalisation. – Secondaire : explorer les facteurs démographiques pouvant influencer la LS de ces patients, comme l’âge, le sexe, le statut d’immigration, le niveau d’éducation et le type d’assurance médicale. Étude observationnelle prospective monocentrique évaluant la LS via le score « STOFHLA » avant l’ETP et deux mois après la sortie d’hospitalisation. Les données démographiques des patients ont été extraites des dossiers médicaux. Quatre-vingt-onze patients inclus (34 transplantés rénaux, 34 pulmonaires et 23 hépatiques) ; âge moyen de 56,6 ± 12,1 ans, avec 40,6 % de femmes. La LS moyenne avant l’ETP et après la sortie d’hospitalisation était respectivement de 26,2 et 32,4 pour les TR, 25,7 et 31,5 pour les TH, et 24,2 et 31,6 pour les TP. Aucune différence significative entre ces groupes avant l’ETP. Les patients étrangers avaient une LS significativement inférieure à ceux nés au Canada (21,8 vs 27,3, p = 0,02). Les diplômés universitaires n’ont pas montré de différence significative de LS (29,3 vs 25,1, p = 0,07). Enfin, une différence significative de LS a été observée entre les patients bénéficiant ou non d’une assurance privée (29,7 vs 22,7, p < 0,01). Avant L’ETP, la LS ne diffère pas d’une population de greffés à une autre. Il est donc essentiel d’offrir le même niveau d’enseignement. La LS a augmenté pour tous les groupes de patients transplantés après l’ETP. Certains facteurs peuvent prédire une LS plus faible, mais il est essentiel de dispenser un enseignement de qualité adapté à chaque patient, sans préjugés.
