A doença de Erdheim-Chester é uma doença sistêmica rara, caracterizada pela infiltração difusa de tecidos e múltiplos orgãos por células do sistema reticuloendotelial, caracterizando uma histiocitose não-Langerhans.Nosso caso ilustra o caráter multissitêmico da doença, com acometimento torácico, abdominal e oftalmológico. HISTÓRICO CLÍNICOPaciente do sexo feminino, 61 anos, com história de tosse seca há 1 mês associada a alteração bilateral da visão.Antecedentes: hipertensão arterial sistêmica, arritmia cardíaca, policitemia vera, hipotireoidismo e insuficiência renal crônica.Nega alergias e cirurgias prévias.Sem antecedente oncológico.Ao exame físico se evidenciou proptose bilateral. ACHADOS RADIOLÓGICOSRealizou-se avaliação inicial de imagem com radiografia simples (RX) de tórax (Fig. 1) que evidenciou obliteração do recesso ázigo-esofágico.Após achados no ecocardiograma, foi indicado estudo de ressonância magnética (RM) de tórax (Fig.2) e do abdome (Fig. 3), que evidenciaram formação tecidual mediastinal envolvendo a aorta ascendente e descendente, tronco braquiocefálico, veia cava superior, átrio direito, veia pulmonar inferior direita e os rins.Para estudo da face, foi indicada RM de orbitas (Fig. 4) que mostrou massa infiltrativa de partes moles retro-orbitária bilateral.Após biopsia do tecido retro-orbitário, o estudo anatomopatológico confirmou o diagnóstico de doença de Erdheim-Chester (DEC).
