Objetivo: Revisar a literatura acerca da relação entre a doença periodontal inflamatória na gestação e a ocorrência de parto pré-termo e baixo peso em neonatos, a fim de discutir os principais resultados das produções científicas já existentes e ressaltar a importância do olhar integral para a saúde da mulher.Metodologia: O presente trabalho consiste em uma revisão narrativa da literatura, em que foi realizada uma busca nas bases de dados PubMed e Scopus, combinando as palavras-chave "Periodontal Diseases", "Premature Birth" e "Infant, Low Birth Weight" com os operadores booleanos "AND" e "OR".Os critérios de inclusão consistiram em artigos com textos completos disponíveis nos idiomas português e inglês, publicados nos últimos dez anos e que apresentavam as implicações da doença periodontal na ocorrência de parto pré-termo ou no baixo peso de neonatos.Duplicatas, cartas ao editor, cartas ao leitor, artigos que não versavam sobre a temática e artigos com textos completos indisponíveis, foram excluídos.Resultados e Discussão: A princípio, 257 artigos foram encontrados, dos quais, 31 estudos foram incluídos.Autores clássicos da Medicina Periodontal, como Offenbacher et al., (1996) e Jeffcoat et al., (2001), também foram citados no presente trabalho.Na maioria da literatura, a presença de bactérias periodontopatógenas no microbioma oral e a ocorrência de doença periodontal inflamatória em gestantes foram associadas ao parto pré-termo e ao baixo peso em neonatos.Citocinas e mediadores inflamatórios atuam na interface materno-fetal e são responsáveis pela ruptura prematura das membranas, o que culmina no parto pré-termo.Estudos divergentes podem ser explicados pelo número reduzido das amostras, além da falta de padronização metodológica.Considerações Finais: As evidências que associam a doença periodontal inflamatória e os eventos adversos na gestação aumentaram, o que ressalta a importância do pré-natal odontológico, essencial para a manutenção da saúde integral do complexo materno-infantil.
