A-Case of a 5-mm Port Site Hernia Occurring After Laparoscopic Sigmoid Colon Cancer Surgery

Abstract
症例は75歳女性．便潜血陽性の精査目的で施行された大腸内視鏡検査にてS状結腸癌を指摘された．既往歴である全身性エリテマトーデスに対してステロイドと免疫抑制剤を内服していた．S状結腸癌cT1b cN0 cM0 cStageIの診断にて腹腔鏡下S状結腸切除術を施行した．第6病日にドレーンを抜去後，第7病日に嘔吐し，各種検査より麻痺性イレウスが疑われたため，イレウス管による腸管減圧を開始した．第10病日に左下腹部痛が出現し，CTにて5mmポートサイト直下にRichter型小腸ヘルニアを疑う所見を認め，緊急手術の方針とした．開腹すると，左下腹部5mmポートサイトの腹壁に小腸が嵌頓していた．嵌頓を解除した後，小腸部分切除術を施行した．術後経過良好にて第39病日に退院となった．今回，5mmポートサイトヘルニアを認めた症例を経験したため，若干の文献的考察を加えて報告する．
