Morbidity and mortality (M&M) conferences were held initially as early as the 1900s by surgeons to scrutinize negative results and identify errors affecting patient care. Traditionally, these conferences have focused on individual error and responsibility.1,2 Although not the initial intent, this approach may cause providers to feel guilt and shame. The literature in psychology demonstrates strong associations between shame and negative coping mechanisms, decreased empathy, and impaired self-forgiveness.
