Tratamento de granulomas laríngeos pós-intubação: revisão sistemática e metanálise proporcional

Introdução: O granuloma laríngeo é uma lesão de contato formada de tecido de granulação no terço posterior das cordas vocais, decorrente de um processo inflamatório que surge após trauma e irritação crônica. Dentre as etiologias traumáticas, intubação e uso prolongado do tubo endotraqueal são as mais comuns. Objetivo: O presente artigo tem por objetivo verificar na literatura os principais tratamentos de granulomas laríngeos pós-intubação. Materiais, sujeitos e métodos: Para a elaboração deste artigo de revisão foram consultados artigos científicos e artigos de revisão publicados e referenciados nas plataformas de dados BVS, Scielo e Pubmed. Resultados e discussão: O tempo para resolução dos granulomas, realizando um tratamento cirúrgico, é o mais rápido dentre todos,visto que resolve imediatamente, apesar de muitos casos precisarem de tratamento clínico, para evitar recidivas. Já a budesonida inalatória é o tratamento que levou um maior tempo médio para a resolução da lesão, e deve ser considerada nas opções para o tratamento de granulomas laríngeos, já que apresenta baixa morbidade. Considerações finais: O estudo permite concluir que as opções de tratamento mais indicadas são conservadoras, com inibidores da bomba de prótons (IBP), glicocorticóides, fonoaudiologia, toxina botulínica, mitomicina C. Caso haja falha terapêutica, cirurgia à laser de CO2 ou instrumentos frios são opções.
