CEREBRAL DYSFUNCTION IN CHILDREN WITH DECOMPENSATED TYPE 1 DIABETES MELLITUS

Abstract
Обследовано 54 подростка, из них 27 пациентов с сахарным диабетом (СД) 1-го типа после перенесенного диабетического кетоацидоза (ДКА) и 27 условно здоровых пациентов. Проведена диагностика церебральной недостаточности по Монреальской когнитивной шкале и нейропсихологический скрининг с целью оценки минимальной мозговой дисфункции. Обнаружено, что пациенты с СД 1-го типа, после перенесенного ДКА имели достоверное снижение когнитивных функций - 22 [21-24] баллов, по сравнению с условно здоровыми детьми 29 [28-30] баллов (р=0,000). Выявлены достоверные нарушения по праксису (р=0,0019) и письму (р=0,000) у пациентов с СД 1-го типа, что говорит о наличие минимальной мозговой дисфункции на этапе декомпенсации заболевания. Показана корреляционная связь между выявленной мозговой дисфункцией, длительностью заболеванием СД 1-го типа и количеством экстренных госпитализаций с ДКА в анамнезе. The study involved 54 adolescents, of whom 27 were patients with type 1 diabetes mellitus (DM) recently emerging out of an episode of diabetic ketoacidosis (DKA), and 27 apparently healthy patients. Participants were tested for cerebral insufficiency using the Montreal Cognitive Assessment tool, and underwent neuropsychological screening to assess minimal brain dysfunction. It was found that the patients with type 1 DM after DKA had a significant decrease in cognitive functions with a score of 22 [21-24] points compared with the apparently healthy children who had a score of 29 [28-30] points (p=0.000). Significant impairments in praxis (p=0.0019) and writing (p=0.000) were found in patients with type 1 DM, which indicates the presence of minimal brain dysfunction during disease decompensation. A correlation has been demonstrated between the identified brain dysfunction, duration of type 1 DM, and the number of emergency hospitalizations for DKA in a patient’s history.
