Several different oesophageal lesions may have a polypoid appearance. These include true neoplasms, both benign and malignant, as well as tumour-like lesions. Inflammatory/hyperplastic polyps are so-called as these polyps typically have a combination of hyperplastic epithelium and inflammation. Inflamed, immature-appearing squamous epithelium may show pseudo-epitheliomatous hyperplasia and regenerative cytological atypia, mimicking squamous-cell carcinoma. Inflammatory/hyperplastic polyps should also be differentiated from squamous papillomas, which have an exophytic, papillomatous surface and much less inflammation. Rarely, atypical inflamed glandular mucosa entrapped within the abnormal stromal cells can mimic adenocarcinoma. Giant fibrovascular polyp should also be differentiated from inflammatory fibroid polyps, which occur rarely in the oesophagus and appear similar to their counterparts elsewhere in the gastrointestinal tract. Leiomyomatosis occurs mainly in adolescents and young adults as a marked, diffuse thickening of the oesophageal wall, with or without nodularity, and extending distally in some cases to involve the proximal stomach.
