Abstract
Les inégalités sociales de santé (ISS) comportent de nombreux défis auxquels tentent de répondre les gouvernements et les acteurs de la société civile. Face aux situations complexes que vivent les populations qui les subissent, des interventions intersectorielles et territoriales constituent une avenue à explorer. Nous proposons un regard sur l’intervention de proximité en centre intégré de santé et services sociaux (CISSS) et en centre intégré universitaire de santé et services sociaux (CIUSSS) ainsi que sa contribution à la réduction des ISS par une pratique agissant sur les déterminants sociaux de la santé. Nous examinons quatre interventions de proximité au Québec pour saisir comment les intervenantes participent à l’identification de ces déterminants contribuant à la réduction des ISS et comment elles interviennent sur ces déterminants. La discussion soulève des enjeux de gouvernance du réseau de la santé et des services sociaux en lien avec le déséquilibre des pouvoirs et l’érosion des services publics en général.
