Abstract
Background: Diabetes mellitus is a chronic metabolic disorder caused by the inability of the pancreas to use insulin effectively. Diabetes mellitus is known as a silent killer which sufferers often do not realize. Patients realize when complications have been discovered. Diabetic neuropathy is a complication or advanced disease that occurs most often and is faced by diabetes mellitus sufferers. Symptoms of neuropathy include burning sensations, vibrations and pain in oneself which is more at night and disrupts the sufferer's sleep patterns. Purpose: To determine the relationship between diabetic neuropathy and sleep quality in people with diabetes mellitus. Method: Quantitative research with correlational design on neuropathy in 63 DM patients at Bendosari Sukoharjo Health Center. The research instruments used were the Toronto Clinical Scoring System (TCSS) questionnaire consisting of 15 questions to measure the level of diabetic neuropathy and the Pittsburgh Sleep Quality Index (PSQI) questionnaire consisting of 19 questions to measure sleep quality. Data analysis used univariate and bivariate Spearman Rho statistical tests to determine the relationship between the two variables. Results: The majority of respondents were male, aged 51-60 years, worked as farmers, suffered for 1-5 years, had blood sugar levels ≥200 mg/dL, underwent treatment with antidiabetic drugs, did not experience neuropathy, and had good sleep quality. The Spearman Rho test showed a p-value of 0.00 (<α 0.05). This indicates a relationship between diabetic neuropathy and sleep quality in DM patients. Conclusion: There is a significant relationship between diabetic neuropathy and sleep quality in DM patients as indicated by the results of the correlation test p-value 0.000 (<0.05). Suggestion: DM patients with diabetic neuropathy should check their blood sugar levels regularly and control them periodically, undergo regular diabetic neuropathy therapy to control symptoms of diabetic neuropathy pain and optimize quality of life. Pendahuluan: Diabetes melitus (DM) adalah gangguan metabolisme jangka panjang yang disebabkan oleh ketidakmampuan pankreas untuk memanfaatkan insulin dengan efektif. Penyakit ini sering kali tidak disadari oleh penderitanya dan dikenal sebagai pembunuh diam-diam. Penderita baru menyadari kondisi mereka saat komplikasi sudah muncul. Salah satu komplikasi yang umum dan sering dialami oleh penderita DM adalah neuropati diabetik. Gejalanya meliputi sensasi terbakar, getaran, dan nyeri pada tubuh, terutama lebih terasa pada malam hari yang dapat mengganggu pola tidur penderita. Tujuan: Untuk mengetahui hubungan antara neuropati diabetik dengan kualitas tidur pada penderita DM. Metode: Penelitian kuantitatif desain korelasional neuropati terhadap 63 penderita diabetes melitus di Puskesmas Bendosari Sukoharjo. Instrumen penelitian yang digunakan adalah kuesioner Toronto Clinical Scoring System (TCSS) yang terdiri dari 15 pertanyaan untuk mengukur tingkat neuropati diabetik dan kuesioner Pittsburgh Sleep Quality Index (PSQI) yang terdiri dari 19 pertanyaan untuk mengukur kualitas tidur. Analisis data menggunakan univariat dan bivariate uji statistik Spearman Rho untuk menguji hubungan antara dua variabel. Hasil: Mayoritas responden berjenis kelamin laki-laki, berusia 51-60 tahun, bekerja sebagai petani, menderita DM selama 1-5 tahun, memiliki gula darah ≥200mg/dL, pengobatan dengan obat antidiabetes, dan tidak ada neuropati serta memiliki kualitas tidur baik. Uji Spearman Rho menunjukkan bahwa nilai p adalah 0.00 (<α 0.05). Hal ini mengindikasikan adanya hubungan antara neuropati diabetik dan kualitas tidur pada penderita DM. Simpulan: Terdapat hubungan yang signifikan antara neuropati diabetik dengan kualitas tidur pada penderita DM, ditunjukkan dengan hasil uji korelasi p-value 0.000 (< 0.05). Saran: Penderita DM dengan neuropati diabetik agar melakukan pemeriksaan kadar gula darah secara teratur dan mengontrolnya secara berkala, menjalani terapi neuropati diabetik secara rutin guna mengendalikan gejala nyeri neuropati diabetik dan mengoptimalkan kualitas hidup.
