Abstract
Le SOPK est fréquemment associé à des troubles métaboliques tels que l’obésité et/ou l’insulino-résistance. Un bilan métabolique est recommandé quel que soit l’IMC. Cependant, la littérature est moins prolifique concernant les patientes avec un IMC normal. L’objectif de cette étude est de caractériser la signature métabolomique des patientes SOPK avec un IMC normal. Nous avons mené une étude rétrospective au sein du service d’endocrinologie de la Pitié-Salpêtrière entre janvier 2019 et octobre 2022 sur les données clinicobiologiques du bilan réalisé en HDJ. Pour identifier la signature métabolomique, nous avons utilisé une approche couplée de spectrométrie de masse et d’apprentissage automatique. Nous avons mis en évidence que l’obésité était majoritairement associée à une insulinorésistance à l’inverse des patientes minces ainsi qu’à des perturbations du bilan lipidique et hépatique. Chez les patientes avec un IMC normal, il a été retrouvé de façon intéressante une élévation des stéroïdes de la voie des delta5. Grâce aux approches de machine learning, nous avons pu confirmer que les paramètres métaboliques étaient discriminants chez les patientes avec une obésité tandis que le profil stéroïdien était discriminant pour les patientes avec un IMC normal. L’obésité dans le SOPK est associée à des troubles métaboliques et notamment à une insulinorésistance tandis qu’on retrouve une élévation des stéroïdes, notamment de la voie des delta5, chez les patientes minces. Il sera intéressant par la suite de comparer notre cohorte à une cohorte de patientes contrôles et d’élaborer un modèle d’IA pour les discriminer.
