Abstract. The need for public service facilities, especially health facilities in the form of hospitals, continues to increase along with population growth. However, hospital construction also has significant environmental impacts from the pre-construction, construction, to operational stages. This impact is related to environmental costs, where the final result depends on the comparison between the value of the costs and the benefits produced. This research aims to identify environmental costs arising from the construction of Salman Hospital from social, economic and cultural aspects. The method used in this research is environmental impact cost analysis (ABDL) and economic valuation analysis. The research results show that the hospital construction area of 18,258 m², which was previously agricultural land, has a negative environmental value of -Rp. 1,172,129,863.98. This value is considered a cost because of the environmental benefits lost due to development. At the construction stage, impacts such as hiring workers and mobilizing tools and materials produce an environmental value of IDR 7,445,597,708.00. Furthermore, at the operational stage, the impact of type C hospital activities and maintenance of infrastructure is estimated to reach IDR 2,625,334,061.28. The total environmental impact of the construction of Salman Hospital reached IDR 8,898,801,905.30, which shows a positive impact on society. Recommendations are given to the project party to pay more attention to potential environmental impacts in order to minimize negative impacts on social, economic and cultural aspects. Abstrak. Kebutuhan terhadap sarana pelayanan umum, khususnya sarana kesehatan berupa rumah sakit, terus meningkat seiring dengan pertumbuhan jumlah penduduk. Namun, pembangunan rumah sakit juga menimbulkan dampak lingkungan yang signifikan dari tahap pra-konstruksi, konstruksi, hingga operasional. Dampak ini terkait dengan biaya lingkungan, di mana hasil akhirnya tergantung pada perbandingan antara nilai cost dan benefit yang dihasilkan. Penelitian ini bertujuan untuk mengidentifikasi biaya lingkungan yang timbul dari pembangunan Rumah Sakit Salman dari aspek sosial, ekonomi, dan budaya. Metode yang digunakan dalam penelitian ini adalah analisis biaya dampak lingkungan (ABDL) serta analisis valuasi ekonomi. Hasil penelitian menunjukkan bahwa area pembangunan rumah sakit seluas 18.258 m², yang sebelumnya merupakan lahan pertanian, memiliki nilai lingkungan negatif sebesar -Rp 1.172.129.863,98. Nilai ini dianggap sebagai cost karena manfaat lingkungan yang hilang akibat pembangunan. Pada tahap konstruksi, dampak seperti penerimaan tenaga kerja dan mobilisasi alat serta material menghasilkan nilai lingkungan sebesar Rp 7.445.597.708,00. Selanjutnya, pada tahap operasional, dampak dari aktivitas rumah sakit tipe C dan pemeliharaan sarana prasarana diperkirakan mencapai Rp 2.625.334.061,28. Total dampak lingkungan dari pembangunan Rumah Sakit Salman mencapai Rp8.898.801.905,30, yang menunjukkan dampak positif bagi masyarakat. Rekomendasi diberikan kepada pihak proyek untuk lebih memperhatikan potensi dampak lingkungan guna meminimalkan dampak negatif terhadap aspek sosial, ekonomi, dan budaya.
