Das innovative Modell der Nurse Practitioner-Led-Clinics in Ontario – Darstellung und Anregungen für die primäre Gesundheitsversorgung in Deutschland

Abstract
Vor dem Hintergrund des wachsenden Mangels an Hausärzt_innen bei einer gleichzeitig zunehmend älter und multimorbider werdenden Bevölkerung stellt die primäre Gesundheitsversorgung eine zentrale Herausforderung dar. Der Sachverständigenrat zur Begutachtung der Entwicklung im Gesundheitswesen und in der Pflege empfiehlt mit dem Fokus Resilienz im Gesundheitswesen den Ausbau regionaler Gesundheitszentren, die bedarfsorientierte Gesundheitsleistungen anbieten. Interprofessionelle Teams, eine Neustrukturierung der Arbeitsteilung und eine definierte Kompetenzerweiterung von hochschulqualifizierten Pflegenden soll angestrebt werden. Seit 2005 wird in Ontario eine solche innovative Versorgungsform, die sogenannten Nurse Practitioner-Led Clinics (NPLC) in der Praxis umgesetzt. Dieses innovative, personenzentrierte, teambasierte Modell ist als Antwort auf bestehende Versorgungslücken entstanden und hat sich mittlerweile in der Primärversorgung etabliert. Der vorliegende Beitrag stellt das Modell der NPLC vor und diskutiert die Potenziale für eine Weiterentwicklung der Primärversorgung in Deutschland.
