Abstract
Abhimanyu et al. investigated how immune stimulation in severe infections links with long-term decreased immune responsiveness through metabolically mediated initiation of DNA hypermethylation at innate immune genes ( 1 ).Importantly, the authors show that blocking this pathway will prevent and potentially restore immune function.This fundamental finding represents therapeutic opportunity for reduction of morbidity and mortality after resolution of severe infections.While severe infections initially lead to strong immune activation, it is often overlooked that this activation may tip toward an immune-tolerant phenotype ( 2 )-the reduced capacity of immune cells to respond to subsequent challenge.Evolution arily, the advantage of tolerance is the prevention of hyperinflammatory phenotypes and autoimmunity, limiting further complications for the host.However, immune tolerance by no means is a cure.While in the short run preventing further inflammation and enabling tissue repair, prolonged cellular tolerance can equally result in increased morbidity and mortality in patients, even after previous successful antimicrobial
