Abstract
The following clinical vignette, in the style of the United Kingdom confidential enquires into maternal deaths, is the start of a series of Brief Communications to be published in the Journal of Obstetrics and Gynaecology Canada. These regular clinical vignettes will contain learning from colleagues working in other provincial and territorial jurisdictions as well. It is important to learn from the tragedies described in these vignettes and hopefully by publishing these cases, all involved in prenatal and postnatal care will learn from these person’s stories and be able to integrate the learning into their day-to-day work.
