Цель исследования – изучить когнитивные функции у пациентов, перенесших COVID-19. Материал и методы. В исследование включено 75 пациентов, перенесших COVID-19 и проходивших лечение в Клинике нервных болезней им. А.Я. Кожевникова по поводу основного заболевания: мигрень, головные боли напряжения, скелетно-мышечные боли в шее и спине. В основную группу вошли 49 пациентов (средний возраст – 48,86 ± 11,85 года), которые помимо проявлений основного заболевания предъявляли жалобы на ухудшение памяти, быструю утомляемость и снижение работоспособности, трудности при концентрации внимания, тревогу, сниженный фон настроения, апатию, возникшие впервые во время, сразу по окончании или в течение 2 месяцев после перенесенной коронавирусной инфекции (КИ), что соответствовало критериям постковидного синдрома (ПКС). Группу сравнения составили 26 пациентов (средний возраст – 42 ± 14 лет), предъявлявших жалобы только в рамках основных заболеваний и не отмечавших каких-либо изменений в их течении после перенесенной КИ. Для объективной оценки когнитивных нарушений (КН) применяли Монреальскую шкалу, концентрации внимания – тест Мюнстерберга, эмоциональных нарушений – шкалу Спилбергера–Ханина и шкалу депрессии Бека, астении – субъективную шкалу оценки астении (MFI-20). Результаты. Пациенты с признаками ПКС характеризуются высоким уровнем личностной тревожности, наличием депрессивной и астенической симптоматики, а также в большей степени субъективными КН, которые представлены преимущественно снижением концентрации внимания и беглости речи. У 27% пациентов выявлены умеренные КН. Пол и тяжесть течения КИ не влияют на наличие КН. Корреляционной связи между выраженностью эмоциональных, астенических нарушений и когнитивной сферой не обнаружено. Заключение. КН – одно из главных проявлений ПКС. Необходимо продолжать наблюдение за пациентами с объективизированными КН, чтобы проследить их динамику, выявить факторы, влияющие на их обратимость или прогрессирование. Objective: to investigate cognitive function in patients after COVID-19. Material and methods. The study included 75 patients undergoing treatment at the A.Ya. Kozhevnikov Clinic of Nervous Diseases for their underlying disease: migraine, tension headaches, musculoskeletal pain in the neck and back, who had recovered from COVID-19. The main group included 49 patients (average age 48.86 ± 11.85 years). In addition to the manifestations of the underlying disease, patients complained of memory impairment, fatigue and decreased performance, difficulties with concentrating, anxiety, low mood, apathy that occurred for the first time during, immediately after or within 2 months after transferred coronavirus infection (CI), which met the criteria for post-covid syndrome (PCS). The comparison group included 26 patients (average age 42 ± 14 years). Patients from the comparison group presented complaints only about their main diseases, they did not note any changes in their course due to the transferred CI. For objective assessment of cognitive impairments, the Montreal scale was used, concentration of attention – the Munsterberg test, emotional disorders – the Spielberger–Khanin scale and the Beck Depression Inventory, asthenia – the subjective asthenia assessment scale (MFI-20). Results. Patients with signs of PCS are characterized by a high level of personal anxiety, the presence of depressive and asthenic symptoms, as well as to a greater extent, subjective cognitive impairment, which is represented mainly by a decrease in concentration and verbal fluency. In 27% of patients, moderate cognitive impairment was detected. Gender and the severity of CI do not affect the presence of cognitive impairment. No correlation was found between the expression of emotional, asthenic disorders and the cognitive sphere. Conclusion. Cognitive impairment is one of the main manifestations of PCS. It is necessary to continue observation of patients with objective cognitive impairment to trace their dynamics, to identify factors influencing their reversibility or progression.
