Abstract
A gentleman with a Glenn repair for pulmonary atresia presented with symptomatic sinus pauses following ablation of an atrial tachyarrhythmia. He had a significant re-entry risk for surgical epicardial pacing and his occupation conferred a prohibitive risk of generator complication with a permanent trans-femoral system. He underwent implantation of a passive fixation leadless pacemaker in the right atrial appendage under ICE guidance. The second deployment, at a more basal appendage site, had satisfactory stability, parameters and performance. This pacing strategy may be considered, with caution, in this challenging population often in need of permanent pacing.
