This chapter deals with how the frequency of congenitally missing teeth differs by location of teeth, gender, and geographic region. It reviews the response of the second deciduous molar when the underlying second premolar is absent, and alternatives for management in the young child. Comprehensive diagnostic records evaluate the patient in all planes of space, establishing a problem list, leading to treatment alternatives and informed consent. The clinician must consider the anticipated longevity of the retained deciduous molar and whether it has become infraoccluded. The retained deciduous molar may fail because of restorative problems, root resorption, or progressive infraocclusion. The vertical alveolar ridge is usually adequate for implant placement, because compensatory eruption of adjacent teeth coincident with skeletal maturation carries the interproximal bone occlusally. The chapter presents various case histories that demonstrate the treatment options available for managing young growing patients with missing second premolars.
