Hip arthroplasty with low concentration local anesthetic fascia iliaca block (HALF) pilot: A case series

Abstract
Suprainguinal fascia iliaca block (SIFIB) provides analgesia for total hip arthroplasty (THA), but with the cost of motor block. Low concentration (LC) local anesthetic (LA) is standard practice in labour epidurals whilst avoiding motor blockade. LCLA SIFIB may be effective, with motor preservation. In this case series of six primary THA patients, pre-operative LCLA SIFIB was performed, coupled with spinal anesthesia. Patients were assessed up to 48 h following spinal for pain scores, opioid consumption and quadriceps weakness. Pain scores were low for all patients. Opioid use was minimal. Quadriceps strength was acceptable for early rehabilitation purposes. In conclusion, LCLA SIFIB may be a motor sparing option for THA.
