Abstract
A 77-year-old man presented with presyncope, dyspnea on exertion, and fatigue 3 weeks after receiving his first dose of atezolizumab, a programmed death ligand-1 (PD L-1) targeted monoclonal antibody, as adjuvant therapy for resected stage II lung adenocarcinoma. His presenting electrocardiogram (ECG) demonstrated normal sinus rhythm with third-degree atrioventricular (AV) block and a slow ventricular escape rhythm of 30 beats per minute (bpm) (Fig. 1, A). The high-sensitivity troponin T was 1644 ng/L and N-terminal pro-B-type natriuretic peptide (NT-proBNP) level was 999 pg/mL.
