Abstract
Despite ongoing advances in the pharmacologic management of pituitary tumors, surgery remains the therapy of choice for a large majority of pituitary tumors. Even though surgical efficacy is now being judged by more rigorous technical standards and by more stringent endocrine criteria than ever before, such scrutiny has only served to reinforce the fundamental role of surgery in the management of pituitary adenomas and other lesions of the sellar region. In this chapter, the surgical management of pituitary tumors is reviewed. Emphasis is placed on indications for surgery, the selection of operative approach, details of operative technique, avoidance and management of surgical complications, and the results of surgery for pituitary adenomas.
