RESUMO Objetivo: Analisar a dinâmica familiar, a rede de apoio de cuidadores familiares de pessoas com câncer progressivo e suas necessidades para a integralidade no cuidado. Método: Estudo qualitativo, descritivo, desenvolvido à luz do Modelo Calgary de Avaliação Familiar. Foi realizado de setembro de 2022 a abril de 2023, por meio de observação participante em instituição pública de saúde de São Paulo e entrevistas com seis cuidadoras familiares. A análise foi realizada a partir do Genograma e do Ecomapa, conforme o Modelo, com auxílio de softwares para organização dos dados. Resultados: Os dados foram categorizados em avaliação familiar estrutural, desenvolvimental e funcional. Revelaram estreitamento de relações familiares com vínculos fortes, aumento do nível de sobrecarga da cuidadora conforme a progressão do câncer, abandono do autocuidado e dificuldades financeiras. As cuidadoras, devido à sobrecarga, tiveram dificuldade de elencar a rede de apoio, mas o modelo empregado possibilitou a identificação dos elementos que a compõem, sendo a fé citada por todas elas. Conclusão: O diagnóstico e a progressão do câncer acarretam mudanças na estrutura, desenvolvimento e funcionalidade familiar, as quais necessitam ser avaliadas individualmente para melhorar o planejamento do cuidado, acionando elementos e serviços ao cuidado de acordo com a realidade das famílias.
