A Lophomoniose pulmonar é uma doença causada por um protozoário do gênero Lophomonas sp, a qual, está frequentemente relacionada a condições de higiene precária e a pacientes imunossuprimidos. As manifestações da doença são diversas e inespecíficas, tornando o diagnóstico desafiador. No presente relato de caso, a hemoptise foi o primeiro sinal que deu início à investigação do quadro. Devido às alterações clínicas e aos achados de exames de imagem, a paciente foi submetida à broncoscopia com lavado broncoalveolar, no qual foi detectado a presença do protozoário Lophomonas sp. Objetiva-se portanto, relatar e relacionar o caso clínico com os dados epidemiológicos e sociais, bem como a relevância do diagnóstico precoce. Este relato reforça a importância da análise diagnóstica ampla, considerando as diversas possibilidades etiológicas nos casos de infecção do trato respiratório que não respondem ao tratamento convencional das pneumonias bacterianas e fúngicas.
