Abstract
L’entorse de la cheville est couramment suivie d’une radiographie qui vise à rechercher l’existence éventuelle d’une fracture. Pourtant, un examen clinique basé sur les règles d’Ottawa permet d’exclure la présence d’une fracture d’un des os de la cheville. Grâce à ces règles, une radiographie n’est nécessaire que lorsque l’examen clinique donne un résultat suspect. L’utilité de la prise en charge « classique » combinant le repos, la glace, la compression et l’élévation de la cheville, n’est démontrée par aucune étude de bonne qualité. À la place d’un plâtre, il est préférable d’opter pour une contention non rigide comme un tape ou une chevillière ou pour une contention semi-rigide comme une orthèse. L’immobilisation de la cheville par une attelle en plâtre ou en résine n’est en fait conseillée que dans certains cas d’entorse sévère. Le Centre Fédéral d’Expertise des Soins de Santé (KCE), en collaboration avec l’UZ Gent, propose un guide de pratique clinique pour le diagnostic et le traitement de l’entorse de la cheville.
