A Case of Very Early Recurrence of Early Gastric Adenocarcinoma with Enteroblastic Differentiation

Abstract
消化器癌において超早期再発は予後不良である．症例は73歳，男性．胃体中部に0-IIc型の早期胃癌を認め，幽門側胃切除を施行．病理所見はtub2＝tub1＞por，T1b（SM2），Ly1b，V1b，N1，HER2陰性でpT1bN1M0 pStage IBの診断で，術後補助化学療法は行わず外来で経過観察を行っていたが，術後6カ月のCTで肝両葉に多発転移を認めた．振り返り病理組織診で，管状腺癌に混じって淡明な胞体を有しGlypican3・SALL4・AFPが一部陽性となる腺癌細胞が確認され，一般型腺癌の一部に胎児消化管類似癌が混在していた．胃原発胎児消化管類似癌は胎児の消化管粘膜に類似した組織形態を示す特殊型の腺癌であり，脈管侵襲が高度で予後不良である．本疾患は稀であり術前診断も困難なため，早期胃癌でも脈管侵襲やリンパ節転移を認める場合にはその存在を念頭に置き，詳細な病理組織学的検索が重要である．
