Abstract

Given the variety and number of caregivers, including the frail person's family and friends, communications and collaboration are essential for a holistic approach to care. Effective communication for collaborative and person-centered care is compromised all too easily by factors such as the lack of an integrated health record, time constraints, a lack of sensitivity to the roles and culture of other professions, inadequate education, and the absence of a treatment coordinator. Moreover, teamwork and shared decision making can be undervalued and viewed as a challenge to each group's sense of professional autonomy. By contrast, good communications include listening, managing information efficiently and with empathy, rapport, ethical awareness, and professionalism in all spoken, written, and virtual interactions. The attention now on person-centered care, interprofessional care teams, and teledentistry coordinated through integrated medical and dental records is aimed at improving care for older patients.
