Impact of lifestyle intervention on clinical outcomes in PCOD: A Systematic Review

Abstract
Background: The three main characteristics of PCOD, a prevalent endocrine disorder affecting women of reproductive age, are hyperandrogenism, ovulatory failure, and polycystic ovaries. Lifestyle therapies, including dietary modifications, physical exercise, and behavioral changes, are often recommended as first-line therapeutic strategies. This systematic investigation evaluates the impact of lifestyle changes on clinical outcomes in women with PCOD. Objective: To compile data on how well lifestyle changes may help women with PCOD with their clinical, metabolic, and reproductive results. Methods: A thorough analysis was conducted to assess the impact of lifestyle changes on clinical outcomes in women with polycystic ovarian disorder (PCOD). To locate relevant studies, a comprehensive search of electronic databases, including PubMed, Scopus, and Web of Science, covering works published up to December 2023, was conducted. The inclusion criteria included observational studies and randomized controlled trials (RCTs) that evaluated the effects of lifestyle interventions, such as diet, exercise, and behavioral therapy, on key clinical outcomes in PCOD, including metabolic parameters, weight control, menstrual regularity, ovulation rates, and hormonal profiles. Two reviewers separately extracted the data, and disagreements were settled by consensus. The quality of the included studies was assessed using standardized tools, such as the Cochrane Risk of Bias tool for RCTs and the Newcastle-Ottawa Scale for observational studies. Results: The review comprised 6 studies. The results showed that lifestyle modifications greatly enhanced the clinical results for PCOD-afflicted women. Combined dietary modifications and structured physical activity were associated with weight reduction, improved insulin sensitivity, and enhanced lipid profiles. Behavioral interventions were effective in promoting adherence to lifestyle changes, resulting in improved menstrual regularity and ovulation rates. Additionally, reductions in androgen levels and improvements in hirsutism were observed. The studies highlighted that personalized and multidisciplinary approaches yielded the most substantial benefits. The comparability of outcomes across research was, however, constrained by variations in study designs, intervention types, and outcome measures. Conclusions: Women with PCOD may improve their clinical, metabolic, and reproductive outcomes with lifestyle changes. Comprehensive and sustained interventions addressing both diet and physical activity provide the most significant benefits. Future research should focus on identifying patient-specific factors influencing intervention success and optimizing strategies for long-term adherence.
