Typical transcranial approaches for parasellar meningioma necessitate a degree of neural retraction, which risks frontal lobe injury or vision loss prompting the uptake of midline endonasal approaches. The transbasal approach seeks to maximize the advantages of each for complete optic canal decompression and tumor control. However, loss of olfaction means that it is typically reserved for cases where olfaction is already affected, the tumor is recurrent, or the tumor is large and complex.
