Топічна діагностика орофаціального болю (клінічний випадок)

Abstract
У статті наведено клінічний випадок 15-річної пацієнтки, яка звернулася зі скаргами на біль у привушно-жувальній ділянці зліва, обмеження відкривання рота, а також іррадіацію болю у скроню, вухо та нижню щелепу зліва. Після первинного обстеження стоматологом-терапевтом пацієнтка була направлена до стоматолога-хірурга для подальшої діагностики. Зважаючи на симптоми та клінічний огляд, було запідозрено міофасціальний синдром больової дисфункції скронево-нижньощелепного суглоба, однак зниження чутливості в ділянці нижньої губи та язика вказувало на можливе ураження трійчастого нерва. Подальше обстеження, включно з електронейроміографією, електроміографією та магнітно-резонансною томографією, виявило ураження лівого нижньощелепного нерва, спричинене наявністю багатокамерного рідинного утворення в проєкції інфратемпоральної ямки, яке тиснуло на нерв. Пацієнтку направлено на консультацію до Інституту нейрохірургії ім. акад. А.П. Ромоданова для подальшого лікування. Цей випадок підкреслює важливість міжфахового підходу в діагностиці та лікуванні орофаціального болю, особливо у випадках, коли клінічна картина виходить за межі типових симптомів міофасціального синдрому. Вчасне залучення спеціалістів різних напрямків дозволяє точніше визначити етіологію симптомів та розробити відповідну тактику лікування.
