Perilaku seksual berisiko yang dilakukan oleh kelompok homoseksual karena sering berganti pasangan dan berhubungan seksual secara anal maupun oral tanpa kondom merupakan faktor risiko yang dapat berkontribusi terhadap penularan infeksi HIV/AIDS di kelompok laki-laki melakukan hubungan seks sesama gender (LSL). Artikel ini bertujuan untuk membahas terkait dampak perilaku seksual berisiko serta strategi pencegahan dan penanganan kejadian infeksi HIV/AIDS akibat hubungan perilaku seks berisiko pada kelompok LSL. Metode penelitian dari artikel ini adalah Systematic Literature Review dengan menggunakan database Google Scholar, Science Direct dan PubMed sesuai dengan PECO serta menggunakan Newcastle Ottawa Scale (NOS) untuk menilai risiko bias. Rentang waktu artikel studi kuantitatif observasional yang dipilih mulai dari tahun 2021–2024 dalam Bahasa Inggris dan Bahasa Indonesia serta memuat kata kunci Perilaku Seksual Berisiko, HIV/AIDS, dan LSL dalam Bahasa Indonesia dan memuat kata kunci Sexual Risk Behaviour, HIV/AIDS, dan MSM dalam Bahasa Inggris yang terdapat naskah lengkap. Artikel diseleksi menggunakan kriteria inklusi dan eksklusi sesuai standar berlaku sehingga diperoleh 10 artikel yang menyatakan perilaku seksual berisiko tanpa pengaman seperti sering berganti pasangan seksual, melakukan hubungan seks anal serta oral tanpa kondom berdampak signifikan terhadap kejadian HIV/AIDS pada kelompok LSL. Oleh sebab itu, diperlukan strategi guna meminimalisir risiko penyebaran infeksi HIV/AIDS untuk kelompok LSL meliputi promosi penggunaan PrEP, penggunaan kondom secara konsisten, tes HIV rutin, intervensi perilaku melalui konseling dan pendidikan, keterlibatan dengan komunitas LSL, serta mengatasi hambatan struktural seperti stigma dan diskriminasi.
