Dans le domaine de la santé, le terme « chronique » est généralement utilisé pour décrire des maladies incurables comme le diabète, l’hypertension ou l’asthme, mais il a également été adopté par des chercheurs et des institutions dans le domaine des dépendances pour caractériser la persistance du trouble d’usage de substances (TUS). Le modèle de maladie chronique du cerveau sur lequel s’appuie cette conception postule que le TUS est une maladie incurable pour l’ensemble des personnes qui en souffrent et qu’en l’absence de traitement, on observe une détérioration de leur condition au même titre que d’autres maladies chroniques. S’appuyant sur tout un corpus de données neuro-médicales indiquant que l’usage prolongé de substances psychoactives entraîne des modifications dans différentes structures du cerveau, les tenants du modèle de maladie chronique attestent que le maintien du TUS dans le temps est expliqué par l’effet de ces modifications sur la capacité d’une personne à exercer un contrôle sur ses comportements. Cet article vise à décrire les fondements théoriques de la notion de chronicité et d’en faire la critique. Après une brève mise en contexte historique, la conception actuelle de la chronicité sera décrite puis critiquée autour de trois éléments : 1) la notion d’incontrôlabilité ; 2) la notion d’irrévocabilité ; 3) la conception monolithique du TUS.
