Abstract
71 歳，男性。初診の約 2 年前より体幹四肢に多発する浸潤性紅斑が蛇行状および環状とさまざまな形状を成していたが，瘙痒などの自覚症状はなく放置していた。近医内科で多血症の疑いで当院の血液内科へ紹介され，皮疹について当科へ紹介され受診した。当科で施行した皮膚生検では膠原線維間にムチンの沈着を伴った組織球の浸潤を認め，interstitial type granuloma annulare の病理組織所見より，汎発性環状肉芽腫と診断した。当科の初診時よりベタメタゾン酪酸エステルプロピオン酸クリーム外用を開始した。血液内科では JAK2V617F 遺伝子変異が検出され真性多血症の診断でヒドロキシカルバミド内服が開始された。初診の約 6 カ月後には皮疹の浸潤は徐々に改善し退色傾向を示した。非典型的な臨床像を示す汎発性環状肉芽腫と診断した場合には，糖尿病だけでなく造血器悪性腫瘍を含めた全身検索も必要であると考える。
