Tóm tắt Đặt vấn đề: Nang ống mật chủ (OMC) là một bất thường giải phẫu bẩm sinh của đường mật trong và ngoài gan. Phẫu thuật cắt nang, tái lập lưu thông qua phẫu thuật nội soi ổ bụng là phương pháp điều trị được lựa chọn. Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu 16 người bệnh nang OMC được phẫu thuật nội soi toàn bộ cắt nang từ 02/2023 đến 07/2024 tại Bệnh viện Hữu nghị Việt Đức. Kết quả: Trong tổng số 16 trường hợp, nữ gặp nhiều hơn nam với tỉ lệ 7:1. Phần lớn không có đủ triệu chứng kinh điển (đau hạ sườn phải, vàng da, sờ thấy khối u bụng) mà chỉ có từ 1-2 triệu chứng. Thời gian phẫu thuật: 227,8 ± 29,7 phút, thời gian làm miệng nối mật ruột: 29,8 ± 8,14 phút, thời gian làm miệng nối ruột ruột: 15 ± 5,6 phút, thời gian hậu phẫu: 8 ± 1,5 ngày. Biến chứng: 01 người bệnh áp xe tồn dư sau mổ chiếm tỷ lệ 6,25% đáp ứng tốt điều trị nội khoa. Không có trường hợp nào rò mật, rò tiêu hóa, tắc ruột, cũng không có trường hợp nào cần can thiệp lại. Kết luận: Phẫu thuật nội soi toàn bộ điều trị nang OMC khả thi, an toàn, biến chứng chấp nhận do ít, nên được áp dụng ở các cơ sở ngoại khoa có trang bị và phẫu thuật viên có kinh nghiệm mổ nội soi. Results of total laparoscopic resection of choledochal cyst in adults at Viet Duc University Hospital Man Van Chung, Nguyen Manh Hung Viet Duc University Hospital Abstract Introduction: Choledochal cyst is a congenital disease of the abnormal anatomy of intra-extra hepatic bile ducts. Total laparoscopic resection of choledochal cyst is the main treatment method. Patients and Methods: Retrospective descriptive study of 16 patients with choledochal cyst were removed by total laparoscopic surgery from 02/2023 to 07/2024 at Viet Duc University Hospital. Results: Among 16 patienst enrolled, female were mostly common than males with a ratio was 7:1. Most patients with choledochal cysts do not have all the classic symptoms (right upper quadrant pain, jaundice, and abdominal mass) but only has 1-2 symptoms. The average surgical time was 227,8 ± 29,7 min, time of hepaticojejunostomy: 29,8 ± 8,14 min, time of Jejunojejunostomy: 15 ± 5,6 min, time of postoperative treatment was 8 ± 1,5 days. The postoperative complication as residual abscess was found in one case, accounting for 6.25%, responding well to medical treatment. There were no cases of bile leakage, digestive leakage, or intestinal obstruction, also no case that required re-intervention. Conclusions: Total laparoscopic surgery for choledochal cysts is a feasible, safe and low-complication surgery that should be widely applied at the surgical facilities well equiped the medical equipment with skilled surgeons.
