Untersuchungen von Komplement in Suralisnerven von Patient:innen mit chronisch entzündlich demyelinisierender Polyneuropathie

Abstract
Einleitung: Die chronisch-entzündliche demyelinisierende Polyneuropathie (CIDP) ist die häufigste immunvermittelte Polyneuropathie, die durch Demyelinisierung von sensorischen und motorischen Nerven zu Einschränkungen führt. Zur Rolle des Komplementsystems bei dieser Erkrankung ist nach wie vor wenig bekannt. Methoden: Unsere Studie analysiert 55 Suralis-Biopsien von Patient*innen mit typischer CIDP und CIDP-Varianten. Die Daten wurden mit nicht erkrankten Kontrollpersonen (NDCs) verglichen. Wir untersuchten auch den Zusammenhang zwischen der kapillaren Komplementablagerung und soziodemografischen Daten, der CIDP-Krankheitsaktivitätsskala (CDAS) und der Erkrankungsschwere gemessen am INCAT-DS und MRC-SS. Resultate: Die Analyse des Nerven zeigte bei 94% der Patient*innen eine Ablagerung des Komplementkomplexes C5b-9 auf endoneuralen Kapillaren, sowie Cluster von T-Zellen und Makrophagen, die bei NCD nicht vorkamen. Genexpressionsstudien zeigten einen Anstieg von C3 und C6 , ohne Unterschiede im Hinblick auf die klinischen Phänotypen. Die Mehrheit der Patienten mit signifikanter Komplementablagerung wies einen progressiven Krankheitsverlauf gemäß CDAS auf, jedoch ohne Korrelation mit dem Schweregrad der Erkrankung. Schlussfolgerung: Unsere Daten sprechen für eine pathophysiologisch wichtige Rolle des Komplementsystems, daher schlagen wir vor, Komplementinhibitionstherapien als neue gezielte Behandlungsoption für Patient*innen mit CIDP weiter zu untersuchen.
