Eine Phase-III-Studie zum Vergleich von Enzalutamid mit der Kombination aus Radium-223 und Enzalutamid bei asymptomatischen oder leicht symptomatischen Patienten mit kastrationsresistentem Prostatakrebs und Knochenmetastasen

Abstract
Ziel/Aim: PEACE-3, eine kooperative Studie von EORTC/UNICANCER/CTI/CUOG/LACOG, untersucht, ob die Kombination von Enzalutamid (ENZ) und Radium-223 (Ra223) (ENZ-RAD) das Fortschreiten des Krebses bei Patienten mit mCRPC im Vergleich zu Enzalutamid allein (ENZ) verbessert (NCT02194842). Methodik/Methods: Männer mit mCRPC und Knochenmetastasen wurden 1:1 auf ENZ oder ENZRAD randomisiert. Ab 03/2018 war die gleichzeitige Gabe von Zoledronsäure oder Denosumab obligatorisch. Der primäre Endpunkt war das radiologische progressionsfreie Überleben (rPFS). Sekundäre Endpunkte sind das Gesamtüberleben (OS), die Zeit bis zur nachfolgenden systemischen antineoplastischen Therapie, die Zeit bis zur Schmerzprogression und die Zeit bis zum ersten symptomatischen Skelettereignis. Ergebnisse/Results: Von 11/2015 bis 03/2023 wurden 446 Männer in die Studie eingeschlossen. Die mediane Nachbeobachtungsdauer betrug 42,2 (IQR 34,3-52,4) Monate (Mo). Die Hazard Ratio (HR) für rPFS betrug 0,69 (95% KI 0,54-0,87; p=0,0009), mit einem medianen rPFS von 16,4 (IQR: 13,8-19,2) Mo im ENZ-Arm und 19,4 (IQR 17,1-25,3) Mo im ENZ-RAD-Arm. Das HR für das OS betrug 0,69 (95% KI 0,52–0,90] p=0,0031) mit einem medianen OS in der vorgeplanten Zwischenanalyse von 35,0 (IQR 28,8–38,9) Mo im ENZ-Arm und 42,3 (IQR 36,8–49,05) Mo im ENZ-RAD-Arm. Während der Behandlung auftretende unerwünschte Ereignisse (TEAE)≥Grad 1 wurden bei 96,4% und 100% gemeldet,≥Grad 3 bei 55,8% und 65,6% der Patienten in ENZ bzw. ENZ-RAD. Kein TEAE≥Grad 3 war im ENZ-RAD-Arm im Vergleich zum ENZ-Arm um mehr als 5% erhöht. Schlussfolgerungen/Conclusions: PEACE-3 zeigt, dass die Zugabe von 6 Zyklen Ra223 zu Enzalutamid als Erstlinientherapie für mCRPC das rPFS deutlich verzögert. Eine Zwischenanalyse, die bei 80% der Ereignisse durchgeführt wurde, zeigt einen OS-Vorteil zugunsten der ENZRAD-Kombination. Die Studie wird fortgesetzt, bis die geplante Endanalyse zur weiteren Bestätigung dieser Ergebnisse abgeschlossen ist. Publication History Article published online: 12 March 2025 © 2025. Thieme.
